Student forms and publications

Read each form carefully for important instructions, processing times and fees (where applicable). For PDF forms or publications, we recommend downloading and then opening in Adobe Acrobat Reader.

Your student record is very important—it is the official record of your course selections, grades and credits earned towards your degree. 

It is very important that you ensure your personal information, such as your mailing address, personal email and telephone number are up-to-date. You can update this yourself through MyOntarioTech, by going to the Personal Information page under the Helpful Links section.

Effective Monday, March 11, 2024, the Office of the Registrar began using the MyCreds™ network for the exchange of official Ontario Tech digital transcripts and degrees. In most cases, official hard copy transcripts will be discontinued.

Through MyCreds™, Ontario Tech will issue official transcripts and digital degrees for students to access and share to academic institutions, employers, government offices and other third parties.

What documents are available through MyCreds™? 

  • Official Ontario Tech University academic transcripts
  • Official Ontario Tech University digital degrees

How do I order an official transcript?

To order an official Ontario Tech transcript, you are required to fill out the request official academic transcript form on MyOntarioTech. Once you have submitted the request and made payment, you will receive an email notification from @mycreds.ca with information about registering for a MyCreds™ account. You will then be able to access and share the document.

How will I receive my digital degree?

Upon meeting the requirements to graduate, your digital degree will be uploaded to MyCreds™. Following the upload, you will be notified via your @ontariotechu.net email that your digital degree is ready.
